COVID-19 Vaccination Status for SBA participants 12+ years

Dear Members,

The National Plan: Phase D 90% (12+ yrs) is now here. This is a reminder that the SBA is now mandating that all people 12 + years of age are now required to show proof of double vaccination on entry to all venues we utilise.

Those that have turned 12 on or after the 1^st of October 2021 will have a grace period but need to send through the details of their first vaccination appointment to admin@southernbasketball.com.au in order to allow entry during their interim vaccination period.

To save a favourite location using your Services Victoria App and QR Code:

Point your Services Victoria ‘check in’ page at the QR code on the poster until you see it on your Phone screen and wait for the prompts. Tap ‘check-in to this location’. You’ll see a confirmation screen with a green tick. At the bottom (left) of the confirmation screen, you’ll see ‘save as a favourite check-in’.

 

Next time you visit this location – no need to check in via the QR code – you can bring up your Favourite  name and “check in”, showing the confirmation screen with green tick to the QR  safety officer
